Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we’re looking for Speech Language Pathologists to join our award-winning team.

Key Responsibilities

• Evaluate patients’ speech, language, and hearing abilities, recommending strategies to enhance communication and hearing.
• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
• Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
• Communicate regularly with physicians, addressing any changes in patient condition.
• Participate in the periodic revision of care plans, updating the physician and care team on patient status and obtaining additional orders as required.
• Educate patients and families / caregivers on speech therapy techniques and related challenges.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Document all evaluation data, treatments, and patient progress promptly.
• Supervise home health aides every 14 days when speech therapy is the sole discipline involved.
