Job Title : Occupational Therapist (Home Health)

Job Summary
Partners In Home Care is seeking a dedicated, compassionate, and motivated Occupational Therapist to join our Home Health team. In this role, youll help patients regain independence and improve their quality of life through personalized, in-home therapy. The Occupational Therapist provides evaluation, treatment, and rehabilitation services, while offering education and support to both patients and families. Youll work closely with an interdisciplinary team to ensure high standards of care, documentation, and compliance.
Essential Duties and Responsibilities
Conduct comprehensive evaluations and develop individualized treatment plans for home-based patients, coordinating with physicians and the care team as needed.Provide skilled, evidence-based occupational therapy interventions and adjust plans as patient needs evolve.Complete and score OASIS assessments accurately, collaborating with the Quality team to ensure compliance.Maintain knowledge of payer guidelines and Medicare Conditions of Participation.Participate in interdisciplinary case conferences and Quality Assurance & Performance Improvement (QAPI) initiatives.Supervise Occupational Therapy Assistants (OTAs) and provide oversight to CNAs when OT is the sole active discipline.Manage patient care throughout the episode, including admissions, 30-day functional assessments, and discharges.Document services and communications promptly and accurately using point-of-care tools.Coordinate daily schedules efficiently to maintain continuity of care and minimize travel time.Maintain current Montana OT licensure and complete all continuing education requirements.Adhere to ethical standards and agency policies while representing PIHCs mission and values.Perform additional duties as assigned by the supervisor.Minimum Qualifications
Bachelors degree or higher from an accredited Occupational Therapy programCurrent licensure to practice Occupational Therapy in the State of MontanaValid drivers license and reliable transportation with appropriate auto insuranceTwo years of occupational therapy experience preferred (home health experience a plus)Schedule
Typical workdays are Monday through Friday, with flexibility depending on FTE status and patient care needs.
