• Recent Home Health experience (2023–2024)
• HomeCare HomeBase experience required
• Blood draw / phlebotomy experience required
• May consider LPNs without HCHB at reduced rate if training is available
• OT must be pre-approved in writing by Administrator

📍 Coverage Area :

Brockton, Abington, Whitman, Rockland, Avon, Holbrook

May stretch to Weymouth & Braintree based on volume

Must be open to assist within a 60-mile radius from the facility as needed
