Lamoille Health Partners , a Federally Qualified Health Center (FQHC) committed to providing comprehensive, patient-centered care to underserved populations, is seeking a dedicated and compassionate Family Medicine Physician (MD / DO), Nurse Practitioner (NP), or Physician Assistant (PA) to join our team.

Responsibilities :
- Provide comprehensive primary care services to patients of all ages, including preventive care, acute care, and chronic disease management.
- Conduct thorough patient assessments, develop individualized treatment plans, and provide patient education.
- Order and interpret diagnostic tests, prescribe medications, and perform minor procedures.
- Collaborate effectively with other members of the healthcare team, including physicians, nurses, medical assistants, and behavioral health providers.
- Maintain accurate and timely patient records in accordance with HIPAA and other regulatory requirements.
- Participate in quality improvement initiatives and contribute to a culture of continuous learning.
Qualifications : Required :
MD / DO, NP, or PA degree from an accredited program.Current and unrestricted license to practice in Vermont.5 years or more of experience working in a health center or hospital setting.Board certification in Family Medicine (MD / DO) or national certification as an NP or PA.Current BLS / ACLS certification.Excellent communication and interpersonal skills.Strong clinical and problem-solving skills.Commitment to serving underserved populations.Ability to work effectively in a team-based environment.Preferred :
Experience working in an FQHC or community health center setting.Experience with electronic health records (EHR) systems, specially eClinicalWorks.Benefits :
