2+ years of Physical Therapy experience

Home Health experience required

Familiarity with Home Health, HCHB, and Oasis systems

Responsibilities :

Provide physical therapy services in patients' homes

Collaborate on individualized care plans

Document progress using EMR

Additional Information :

Potential for overtime and contract extension
