Behavioral Health Clinical Social Worker - Part-time or Full-time and Hybrid

Location : Westlake

Schedule : M-F (No evenings or weekends)

Compensation : $60,000 annually

Our team is currently seeking a Behavioral Health Clinical Social Worker within the Family Support Services Department, part-time or full-time and hybrid schedule is available. The employee will have the capability to work from home 2 to 3 days a week.

Responsibilities :

• Provides psychosocial assessments, including ability to diagnose psychological, behavioral and emotional disorders.
• Conducts screenings through client / family interviews.
• Provides behavioral health counseling and interventions based on client’s needs identified within Individualized Service Plan.
• Collaborate with internal and / or external service providers to support progress toward identified goals and objectives within Individualized Service Plan.
• Facilitates community linkages, outreach, education, advocacy and networking to promote and enhance behavioral health programming and services.
• Provides consultation, education, advocacy, and community outreach services, as requested.
• Completes any / all required documentation accurately within expected timelines.
