Responsibilities :

• Provide individualized treatment plans to help patients with ADLs (activities of daily living)
• Perform assessments and recommend adaptive equipment as needed
• Collaborate with interdisciplinary care teams (RNs, PTs, Social Workers)
• Document visits accurately using EMR (WellSky, Kinnser, or similar)
