• Follow a designated care plan in accordance with patient's needs\n
• Make home visits to clients in designated geographic territories\n
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures\n
• Monitor clients' conditions; reporting changes to Clinical Manager\n
• Follow up with, execute, and properly document doctors' orders\n
• Case management and coordination\n
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a touch pad tablet\n\nQualifications for a Registered Nurse (RN) : \n
• A current license as a Registered Nurse in Massachusetts\n
