We are seeking a compassionate and dedicated Direct Care Professional (Part-Time Weekends) to provide support and assistance to individuals with disabilities, chronic illnesses, or aging-related needs in a residential or community-based setting. The ideal candidate will have experience working with vulnerable populations and possess excellent interpersonal skills to foster a supportive and empowering environment.

Responsibilities :

• Assist clients with daily living activities, including personal hygiene, meal preparation, and light housekeeping.
• Provide emotional support and companionship to clients, engaging them in social and recreational activities.
• Administer medications and monitor clients' health and well-being, reporting any concerns to supervisors or healthcare professionals.
• Assist clients with mobility and transportation needs, including transfers and wheelchair assistance.
• Maintain accurate and confidential client records, documenting daily activities and progress.
• Encourage client independence and support skill development through individualized care plans.
• Communicate effectively with clients, family members, and other healthcare professionals.
• Participate in ongoing training and professional development to improve skills and knowledge in the field.
