Job Summary :

Familyway Home Care is seeking a compassionate and experienced Perdiem Case Manager / Registered Nurse (RN) to join our team. This role is responsible for coordinating patient care , ensuring compliance with state and federal regulations, and providing oversight to home health aids. The Case Manager / RN will serve as the primary point of contact for clients and families, developing and implementing care plans to meet each client's unique needs.

Key Responsibilities :

Patient Care Coordination :

• Assess, develop, and oversee individualized care plans for clients receiving home care services.
• Conduct initial and ongoing assessments of patients to evaluate their health status and home care needs.
• Monitor and ensure the quality of care provided by home health aids.
• Provide education to patients and families on health conditions, medications, and self-care techniques.
