Shift : 8a - 5p

Schedule : M-F 2 weeks a month

August 8 to August 26

August 31 to September 8

September 15- 23

September 27 to October 14

October 24 to November 4

November 14 to Dec 13

December 21 to Jan 9 Must cover

Call Schedule : 10 + on call days per month (reduced clinic day schedule if overnight births)

Scope : 85% outpatient and 15% inpatient mix

Requirements (BC / BE, vaccine, license, NPDB) : BC / BE, MI & Compact, Clean NPDB (TBD on vaccine (medical and religious)

EMR : E-clinical works in clinic & Meditech in Hospital

Volume : 16 patients per day, Births per month 16 to 19, 3 to 5 C-sections per month

Group / Hospital : UPS in Portage, Level III trauma center, ACS verification Status

Support : Staffed with RN, LPN and MA
