We are searching for a Director of Social Services to lead our social services department and ensure residents, and their families receive the support they need. We recognize the vital role that social services plays in improving the well-being and quality of life for those in our care and are looking for a team builder who is excited about the opportunity to assist in building a facility culture. The primary purpose of the role is to be responsible for providing services to respond to the emotional needs of the residents and their families. You will also assist with resident admissions and the referral process.

Major responsibilities include staying up to date with federal and state regulations, implementing policies and procedures to identify and address the social and emotional needs of residents, participating in community planning, overseeing discharge planning, developing social care plans, and providing consultation to staff and community agencies to develop effective social service programs.

Minimum qualifications include a Bachelor's or Master's degree in Social Work, Human Services, or a related field, experience in long-term care, healthcare, or social services within a skilled nursing or similar setting, strong understanding of state and federal regulations governing social services in nursing facilities, excellent interpersonal, communication, and problem-solving skills, and the ability to build and maintain relationships with residents, families, and external agencies.
