Key Responsibilities

• Conduct comprehensive in-home evaluations and develop tailored treatment plans.
• Deliver one-on-one physical therapy focused on mobility, balance, and strength.
• Educate and empower patients and caregivers on safety, exercise programs, and recovery strategies.
• Collaborate with nurses, OTs, social workers, and physicians to provide coordinated care.
• Maintain timely EMR documentation while adhering to compliance and safety standards.
• Travel between patient homes within your territory; mileage reimbursement and flexible scheduling provided.

Requirements

• Active Oregon Physical Therapist license (Doctorate or Master’s preferred).
• Open to new graduates; home-health experience preferred but not required.
• Current CPR / BLS certification.
• Reliable transportation with valid driver’s license and insurance.
• Excellent communication, documentation, and problem-solving skills.
