Job Summary :

The Manager for Case Management services reports to the Case Management Director and is responsible for promoting patient-centered care by coordinating all aspects of hospital -based case management for his / her assigned area of responsibility in alignment with the goals of the Case Management Department.

Qualifications / Requirements :

• Associate's / Bachelor's Degree in Nursing OR Master's Degree in Social Work
• 3+ years' experience in Acute-Care Case Management or Social Work
• 2+ years of supervisory experience
