- Manage and coordinate care for patients admitted to the hospital
- Conduct thorough patient evaluations, develop treatment plans, and monitor progress
- Collaborate closely with consulting specialists and the broader care team
- Lead discharge planning and ensure safe transitions to outpatient care
- Participate in hospital quality improvement initiatives and clinical protocols
- Maintain compliance with hospital policies, regulatory requirements, and best practices
