Essential Job Functions & Responsibilities :

• Providing direct patient care under the supervision of the Hospice Clinical Manager.
• Makes home visits as necessary. Works cooperatively with patients, families, nursing staff, physicians and ancillary service providers.
• Conducts patient triage from patients / families.

Requirements : Job Qualifications :

Pay range : $32-$35 / hour
