Pay Information $2,406 per week About The Position Job Title : Home Health & Hospice RN Profession : RN (Registered Nurse) Specialty : Home Health & Hospice Duration : 13 weeks Shift : Days Call Hours per Shift : 9 hours Experience : 1 year License : Required Certifications : BLS Must-Have : - Home Health & Hospice experience - Current Driver's License and Auto Insurance Description : Responsible for assessing client and family home health needs. Completes physical assessments and history of current and previous illness. Regularly re-evaluates client nursing needs. Utilizes health assessment data to determine nursing diagnosis. Initiates and revises the plan of care as client status and needs change. Develops care plans and establishes goals based on diagnosis. Incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and family in the planning process. Administers medications and treatments as prescribed by the physician. Provides health care instructions to the client as appropriate. Identifies discharge planning needs as part of care plan development. Acts as Case Manager when assigned by Clinical Supervisor. Coordinates client care for assigned caseload. Prepares clinical notes and updates the primary physician as necessary. Communicates with the physician regarding client needs and reports changes in condition.
