What You’ll Do :

Provide outpatient care to a panel of adult patients (primarily Medicare population)

Focus on prevention, chronic disease management, and wellness

Collaborate with care teams to address medical, behavioral, and social health needs

Maintain high-quality standards and documentation with EMR support

Build trusted relationships with patients over time

What You’ll Bring :

Board-certified or board-eligible in Family Medicine or Internal Medicine

Louisiana medical license (active or in progress)

Strong communication and decision-making skills

Interest in long-term care relationships and practice transformation
