Responsibilities :

Home Visits : Build trusting relationships with unique bonds through regular home visits.

Comprehensive Assessments : Evaluate physical, psychosocial, and environmental needs in the comfort of the patient’s home.

Education & Communication : Empower patients and families with knowledge and recommendations to enhance their quality of life.

Team Collaboration : Work cohesively with other healthcare professionals to deliver top-notch care.

Documentation & Supervision : Ensure timely paperwork submission and provide guidance to aides for optimal patient care.

Qualifications :

Nursing Degree : From an accredited institution (Diploma, Associate, Bachelor's, or vocational nursing).

Active RN License : Current state license as an RN.

CPR Certified : Must provide proof of current certification.

Reliable Transportation : Current driver's license and car insurance.

Soft Skills : Compassionate, excellent communicator, friendly demeanor

Experience : 1 year RN home care experience (Preferred)
