Femoral Popliteal Bypass

Aortofemoral Bypass

Stenting

Endovascular and Open Surgical Reconstruction for Deep Vein Occlusions

Treatment of

Vascular Malformations

Nutcracker Syndromes

Lymphedema

Gangrene of the limbs

Peripheral Artery Disease

SUBMISSION REQUIREMENTS

Board Certified Vascular Surgeon - REQUIRED

Fellowship Training - REQUIRED

OH License - Preferred , willing to accept providers needing to obtain OH license ( Note status at time of name clear)

BLS & ACLS Certified - REQUIRED

COVID Vaccine - REQUIRED , noted at time of name clear

Other Info :

The provider will be covering 4 locations

Practice Setting : Hospital, Will not require clinic hours

Average Number of Cases Per Day : Rounding at 4 hospitals with an average of 6 patients at each hospital

Documentation System / EMR : EPIC

Trauma Level / Designation : 1

Number of Operating Rooms : Each hospital has multiple

Patient Age Demographic : Adult

Minimum Board Certification : BC

EMR System : Epic
