Licensed Practical Nurse (LPN) - Home Care Services

Job Summary : Join our dedicated team as a Licensed Practical Nurse (LPN) providing compassionate home care services. In this role, you will deliver quality nursing care consistent with the plan established by the RN Case Manager.

Key Responsibilities :

• Nursing Care : Deliver intermittent skilled nursing services while assessing the physical and psychosocial status of patients.
• Documentation : Accurately document all services within 24 hours, including all physician orders and any changes to the care plan.
• Collaboration : Communicate regularly with the RN Case Manager and maintain flexibility for unexpected patient visits.
• Professional Development : Commit to ongoing education with at least 12 hours of in-service training per year, actively seeking out development opportunities.
• Compliance : Adhere to HIPAA regulations and Agency policies, ensuring the confidentiality and safety of patient information.

Conditions : Work involves some exposure to communicable diseases and requires the ability to perform physical tasks. A valid driver's license and adequate auto insurance are required.
