What You'll Do

Provide in-home health assessments and create personalized care plans

Administer medications, treatments, and wound care as prescribed

Educate patients and families on health management and independence

Monitor progress and update care plans as needed

Supervise LPNs and Home Health Aides for quality assurance

Participate in interdisciplinary care meetings to ensure coordinated care

What You Bring

Active Arizona RN license

At least 1 year of RN experience (home health, hospice, or critical care preferred)

CPR certification

Valid driver's license and reliable transportation

Strong clinical, communication, and organizational skills
