Reports To : Supervising Care Manager (SCM)

Summary : Responsible for overseeing the DAS Case Management program and providing case management services to seniors and adults with disabilities.

Essential Functions :

• Assist SCM to oversees the Case Management Program and Short-Term Home Care for the Older Adults program with DAS.
• Trains, supervises, and evaluates the program's staff.
• Provides case management services to seniors and adults with disabilities, which include :
• a. Conducting intake;
• b. Completing a comprehensive assessment of the consumer's functional and cognitive capacity and limitations during a home visit;
• c. Developing a care plan based upon the assessment;
• d. Implementing the care plan by coordinating and arranging for services;
• e. Following up and monitoring the plan to ensure that the care plan continues to meet the consumer's needs;

f. Reassessing the client's needs on a regular basis; and

g. Terminating case management services when appropriate.
