Job Description
Metro is hiring a motivated and passionate Per Diem Supervising Physical Therapist to oversee care provided by PTAs in the Center Moriches area. The PT's primary responsibility will be to perform initial evaluations, progress notes, and required supervisory visits for PTAs. This is a high-need area with an immediate need for PTA supervision coverage. There is also the option to take on ongoing treatment visits in addition to the supervisory role, depending on your availability and interest. This is a flexible per-diem position, with competitive per-visit rates for both evaluations and supervisory visits.

Accept and treat patients within your designated geographic areaPerform initial evaluations, progress notes, and required supervisory visits for PTAsEvaluate patients' needs for physical therapy in the home setting and develop individualized treatment plansDevelop a plan of care and treatment goals in consultation with the physician, patient, and familyPlan, prepare and implement customized therapy programs to restore function and promote independenceInstruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniquesProvide timely and accurate documentation for each visitCommunicate effectively with patients, families, and interdisciplinary team members, especially around discharge planningDeliver care in compliance with agency policies and professional practice standardsQualifications
Physical Therapist Program GraduateLicensed Physical Therapist in the state of New York or eligible.Mandatory Certifications : First Aid, AED, & CPR Certifications.Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.Ability to travel to patients' homes within the assigned geographic territoryAdditional Information
