Our client is currently seeking a Registered Nurse Case Manager

Location : Lake County, CA (Field-Based)

Schedule : Day Shift | 40 Hours / Week

Travel : Required within coverage area (mileage reimbursed at government rate)

Role Overview

As a Home Health RN Case Manager, you’ll lead the coordination of care for patients in their homes, ensuring timely, appropriate, and high-quality services. You’ll serve as a clinical liaison among patients, families, physicians, and interdisciplinary teams, driving optimal outcomes and continuity of care.

What You’ll Do

• Lead interdisciplinary care planning and ensure alignment with clinical needs and covered services
• Monitor diagnostics, treatments, and procedures for appropriateness and timeliness
• Collaborate with physicians on resource utilization, treatment plans, and discharge strategies
• Coordinate transfers to tertiary centers and ensure continuity of care
• Manage complex cases including frequent ED utilizers, chronic pain, substance abuse, and homelessness
• Collect and report data for regulatory and accreditation compliance
• Perform other duties as assigned
