Job Description :
The clinical staff pharmacist will work in both the home infusion and specialty pharmacy department and will be responsible for providing safe and effective medication and services in accordance with AleraCare Pharmacy California's policy and procedures, state and federal regulations. The clinical pharmacist works closely with the intake department, nursing agencies and monitors patient progress toward goal.
This is a Monday through Friday position.
Job Responsibilities include but are not limited to :
- Determine a patient's suitability for home care services based on information provided from the referral source.
- Recommend, monitor and evaluate patient specific plans of care all home care patients. When appropriate, pharmacist will make recommendations for medication modifications.
- Competence in assessing lab values and pharmacokinetic principles in order to monitor and dose medications per protocol, as appropriate / applicable.
- Provide medication education – consultation to patients, staff, other healthcare professionals as needed / requested.
- Verify orders with prescribers' offices or obtain orders as needed.
- Maintain good communication with patient / caregivers, intake department, home health agencies and physician offices as needed to provide continuity of care and to ensure a clear understanding of plan of care while on service with AleraCare Pharmacy California.
- Prepare and dispense medication following AleraCare Pharmacy California's policy and procedures. He / she will adhere to the standards of USP 797 and USP 800, DEA, California Board of Pharmacy, accreditation organizations, and any other regulatory agency.
- Ensure confidentiality is maintained at all times.
- Rotate being on-call

Basic Education / Experience Requirements and Preferred Qualifications :
Graduate of an accredited school of pharmacyCurrent, active pharmacy license granted by California State Board of PharmacyPharmacist must have at least one year of experience with IV sterile compoundingHome infusion experience is preferredSpecialty pharmacy experience is preferredBasic level of knowledge with Microsoft Word, Excel and PowerPointGood interpersonal skills and clear communication skills to effectively speak with other healthcare professionals and patientsPhysical Requirements :
