About The Position

Description :

In collaboration with patients and families, case managers, physicians, and the interdisciplinary team, the Clinical Social Worker provides direct psychosocial and practical care.

This includes social work assessments, problem identification, goal setting, behavioral intervention, and implementation of care plans.

The Clinical Social Worker collaborates with all disciplines to optimize patient outcomes.

Scope of services may include developing action plans to mitigate psychosocial barriers to discharge.

This position involves complex post-acute care coordination, patient and family engagement, and education.

The Clinical Social Worker employs clinical interventions to address immediate needs.
