Key Responsibilities :

• Conduct thorough patient assessments and complete accurate OASIS documentation
• Create and implement patient-specific care plans to meet individualized needs
• Provide nursing care, including wound care, medication management, and health monitoring
• Collaborate with a multidisciplinary healthcare team to ensure continuity of care
• Educate patients and families on health maintenance and self-care techniques
