Job Summary

We are seeking a dedicated and compassionate QM Specialty NP / PA to join our facility in Inverness, IL. As a QM Specialty Provider, you will provide comprehensive primary care services via telehealth to a diverse patient population. This is a full-time position ideal for a provider who values meaningful patient relationships and a collaborative care environment.

Responsibilities

• Conduct assessments, diagnose, and develop treatment plans tailored to patient needs
• Manage chronic diseases and acute conditions
• Collaborate with interdisciplinary teams to enhance patient care
• Maintain accurate and timely medical records in compliance with documentation standards.
• Assist with quality management (QM) initiatives, including compliance audits, clinical evaluations, and outcome tracking
• Ensure adherence to regulatory guidelines and clinical protocols
• Provide guidance and support to healthcare team members regarding best practices and performance improvement initiatives
• Perform physical exams and medical history assessments
• Collaborate with facility staff and interdisciplinary teams
• Educate patients and families about treatment options and care goals
• Ensure a safe, respectful, and supportive environment for residents
