Job Description Responsibilities Assessing patients for hospice eligibility and explaining hospice services to patients and families. Developing and implementing individualized care plans. Providing direct patient care such as pain management, wound care, and assistance with daily activities. Supporting patients emotionally and guiding families in caregiving roles. Collaborating with the interdisciplinary team for holistic care. Adhering to company values of compassion, integrity, excellence, teamwork, and innovation. Maintaining documentation and compliance with regulations. HCHB and Hospice Experience Required. A current driver's license. Current CPR certification or Basic Life Support (BLS) certification.
