As a Physical Therapist, you can expect : The ability to build in-person, trusted therapist-patient relationships.

Physical Therapist (PT) Duties and Responsibilities :

• Adheres to CMS guidelines by providing initial and ongoing assessments to determine the level of functioning, including OASIS assessment at appropriate time points.
• Gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psychosocial functioning.
• Develop and revise the plan of care in consultation with the physician and other care team members.
• Responsible for planning, evaluation, and treatment implementation of an assigned caseload.
• Preparing clinical progress notes and evaluating outcomes.
• Supervising therapy assistants and home health aides as appropriate.
• Educate patients and / or families about proper safe use and recommend methods for the installation of adaptive equipment.
