Join a dedicated healthcare team providing post-acute care in community facilities, working alongside Nurse Practitioners and Physician Assistants to enhance patient outcomes.

This Advanced Practice Provider role involves managing and coordinating patient care, conducting rounds, documenting in electronic medical records, and collaborating with staff and physicians.

Candidates need strong communication skills, empathy, teamwork, organization, and problem-solving abilities. A Nurse Practitioner license and DEA certification are required, with preferred experience in geriatrics, internal medicine, or emergency medicine.
