MUST have active NJ license, DEA and CDS Max bill rate is $300 per hourCoverage Dates Needed- Saturdays 24 hours- 7a-7a, or 7p-7a Sundays 24 hours- 7a-7a or 7p-7aPlease list any anticipated procedures / surgeries the provider will be required to perform and anticipated volumes : Labor Epidurals, Spinal Anesthesia, C-sections Description of Work Assignment : Weekend Obstetrics Coverage Breakdown of Clinic Days / Sessions and / or OR Days : Weekend OB coverage for busy OB practices Schedule of Days / Times Needed to Work / Shift Hours : Saturdays and Sundays On call? Shift work Patient Load : Est. 7000 deliveries annually Board Status Requirement (BC / BE) NBCRNA Board certification, NJ Nursing License, DEA, CDS, BLS, ACLS, PALSIs there additional support staff? Attendings on call, other CRNAs Is the provider working solo or with other providers? If so, whom? See above Trauma Level of Work Location : Level IV trauma center EMR System?
