Float Location : Worthington, MN (2 days per week)

Guaranteed Hours : 37 hours per week (GWR)

Job Description

We're hiring a motivated and compassionate Physical Therapist (PT) for a contract assignment in a Skilled Nursing Facility (SNF) setting located in Lamberton, Minnesota . This position includes scheduled floating to Worthington, MN two days per week.

The ideal candidate is a dependable Travel PT or Contract Physical Therapist who enjoys working in a collaborative rehab environment and providing high-quality patient care to an older adult population.

Responsibilities

Evaluate, plan, and deliver physical therapy treatments to patients in SNF and sub-acute rehab settings.

Create individualized treatment plans to help residents improve mobility, strength, and functional independence.

Accurately document all therapy sessions, progress notes, and discharge plans per facility and regulatory requirements.

Work closely with occupational therapists, speech-language pathologists, nursing staff, and physicians to coordinate patient care.

Maintain an 85% productivity standard .

Travel between Lamberton and Worthington facilities as scheduled.

Qualifications

Current and valid Physical Therapist (PT) license in the state of Minnesota (or eligibility for MN licensure).

Graduate of an accredited Physical Therapy program.

Experience in a Skilled Nursing Facility (SNF) or geriatric rehab preferred.

Excellent communication, organization, and time management skills.

Reliable transportation for float days.
