Must-Have : Candidate must be reviewed and pass clinical assessments, have a flexible schedule including working major holidays as needed.

Job Description :

• Assess, evaluate, and drive the discharge plan.
• Coordinate services to match the patient's appropriate level of care.
• Ensure that the treatment plan is initiated, evaluated, and reassessed daily.
• Monitor progress or lack thereof on a daily basis.
• Track the length of stay and identify barriers to discharge.
• Follow Medicare regulatory guidelines regarding patient choice, IMM notices, and MOONS.
• Work schedule : Monday through Friday, with no weekend or on-call requirements.
• Floating within facility / scope of practice as needed.
• Compliance is required for an on-time start, with a possible two-week delay for non-compliance.
• The candidate must be a traveler away from certain facilities for one full year to reapply.
• Supervisory references are highly preferred.
• Candidates must be available to submit core documents and complete / schedule labs within 48 hours of contract signing to ensure an on-time start.
