A company is looking for a Case Manager / RN (licensed in all 50 states).
Key Responsibilities
Manage individual caseloads and communicate with healthcare team members to meet patient needs
Review medical data and develop case management plans, documenting goals and actions
Maintain case files, prepare reports, and ensure continuity of care through ongoing patient contact
Required Qualifications
Current active and unrestricted Registered Nurse license in all 50 states
Bachelor's degree in a health-related field preferred
7 years of varied clinical experience preferred
URAC-recognized certification in Case Management within 3 years of hire
Proficiency in using computer systems for case documentation and billing
