California Licensed Clinical Coordinator

A company is looking for a Clinical Coordinator (RN).

Key Responsibilities

Support quality patient care by collaborating with the Care Team and Director of Nursing

Handle clinical inquiries from patients and staff, escalating as necessary

Provide oversight and care coordination for a designated caseload of patients

Required Qualifications

Active California RN BSN License

1-2 years of experience in Home Health

Current CPR Certification

Knowledge of intake, scheduling, authorizations, OASIS, and orders (preferred)
