Required Licensure :

  • Active RN Licensure in New York State

    • Required Experience :

  • Minimum 2 years of HOME setting experience within the last 15 years
  • Must have recent experience conducting comprehensive patient assessments

    • Important Notes :

  • This is a hybrid role

    • Telehealth + in-field UAS assessments

  • Candidates cannot perform UAS assignments elsewhere
  • Will cover specific zip codes not the entire county

    • Job Summary

    Nurses conduct initial UAS assessments, evaluating health status, strengths, care needs, and preferences to guide individualized long-term care service plans. The role requires excellent organizational, communication, and clinical skills, with the ability to work independently in a fast-paced environment.

    Job Responsibilities

  • Conduct in-home and telehealth pre-evaluations of patients, including UAS assessments.
  • Review consumer medical documentation and referral forms.
  • Enter evaluation data into electronic forms and transmit as required.
  • Document concerns, discrepancies, or issues identified during assessments.
  • Support continuity of care, reducing fragmentation and duplication.
  • Discuss healthcare options, supports, vendors, and waiver programs with consumers.
  • Coordinate with the Call Center to enroll eligible consumers into managed care plans.
  • Foster collaboration with interdisciplinary care teams to enhance service delivery.
  • Collect quality review data and case notes in CRM systems.
  • Promote health and illness prevention opportunities.
  • Maintain knowledge of community resources, payor requirements, and services.
  • Meet all standards outlined in performance criteria and bonus templates.
  • Perform other duties as assigned by the Regional Nurse Manager or project leadership.
