Licensed Practical Nurse (LPN / LVN) – Day Shift
Facility : Albuquerque Heights Health Care
Area : Albuquerque, NM
Class : LPN
Grade : 15
Shift : 1st Shift – 07 : 00 AM – 7 : 30 PM (12-hour shifts)
Position Summary
Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides safe, efficient, and compassionate nursing care to patients while achieving positive clinical outcomes and patient / family satisfaction. The LPN practices within the scope defined by the State Nurse Practice Act , contributes to care planning and evaluation, delivers direct patient care, and supervises unlicensed assistive personnel in the delivery of daily nursing activities.
Key Responsibilities 1. Patient Evaluation
Collect, document, and report objective and subjective patient data.
Observe and report any changes in patient condition to the RN promptly.
2. Care Planning
Contribute to developing individualized patient goals and care plans.
Assist in creating and implementing interventions to meet patient goals.
Evaluate the effectiveness of nursing interventions and adjust as needed.
Participate in reviewing and revising the plan of care.
3. Direct Patient Care
Administer medications and treatments per physician orders and facility protocols.
Accurately document all care provided in Point Click Care (PCC) .
Communicate patient information effectively to RNs and physicians.
Assist physicians during rounds and patient examinations.
Participate in Utilization Management and Interdisciplinary Care Management meetings to support discharge planning.
Educate patients and families regarding care and discharge instructions.
4. Supervision of Unlicensed Staff
Assign duties in accordance with the State Nurse Practice Act, Genesis policy, and staff training.
Communicate patient information and care priorities during and between shifts.
Monitor, evaluate, and follow up on delegated tasks to ensure quality and compliance.
Maintain accountability for care delivered by unlicensed personnel.
5. Additional Responsibilities
Perform shift-to-shift narcotic counts and maintain medication security.
Ensure effective communication and teamwork across departments.
Support a culture of safety and respect for staff, patients, and families.
Assist in training and orienting new nursing staff.
Maintain patient confidentiality and protect Protected Health Information (PHI).
Work beyond scheduled shifts when needed to meet state staffing or patient care requirements.
Participate in required in-service programs and continuing education.
Perform other duties as assigned.
Compliance and Ethics
Adhere to all federal, state, and facility regulations, including HIPAA and the Federal False Claims Act.
Participate in compliance training and report any suspected non-compliance through appropriate channels.
Support quality assurance and process improvement initiatives.
Cooperate fully with internal audits, investigations, and monitoring functions.
Qualifications
Graduate of an accredited Practical or Vocational Nursing Program ( Required ).
Active LPN / LVN license in the State of New Mexico ( Required ).
BLS / CPR certification – Current and valid ( Required ).
Experience : Preferred background in rehabilitative or geriatric nursing .
Must be able to read, write, speak, and comprehend English fluently to ensure patient safety and effective communication.
Must provide proof of tuberculin skin test (TST) as required by state law and company policy (administered at work site if necessary).
