Washington State Licensed Case Manager

A company is looking for a Case Manager.

Key Responsibilities

Engage target members facing medical, mental health, and substance utilization disorders to build supportive relationships

Conduct initial holistic health assessments to identify member needs and barriers to lifestyle changes

Develop and monitor member-centric case management plans in collaboration with members and healthcare providers

Required Qualifications

Current Registered Nurse (RN) state license

3 years of experience in direct patient or clinical care post-licensure

High-speed internet connection (minimum 20 Mbps)

Must obtain Certified Case Manager (CCM) certification within 24 months if not already certified

Valid WA state or MSL / Compact license required
