We are seeking a skilled Physical Therapist to join our team in delivering exceptional care to patients. As a Physical Therapist, you will be responsible for assessing the needs of our patients and developing personalized treatment plans tailored to their requirements.

You will provide therapy services including creating and administering care plans, overseeing physical therapy assistants, and collaborating with the interdisciplinary care team, physicians, and other stakeholders in patient care.

Your key responsibilities will include :

• Determining physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition.
• Providing and completing all necessary documentation.
• Administering physical therapy treatments as ordered by a physician and directing and overseeing treatment given by physical therapy assistants.
• Evaluating the effects of physical therapy treatments and the fit of prosthetic and orthotic devices by observing, noting, and evaluating patients' progress, and recommending adjustments and modifications.
• Completing discharge planning by consulting with the care team and physicians and ensuring any appropriate continuation of the therapeutic plan post-discharge (e.g., home exercise programs; recommending and / or providing equipment; recommending outpatient follow-up programs).
• Participating in weekly case conferences and effectively communicating about patient care across disciplines involved.
