We are looking for dedicated and experienced Registered Nurses (RNs) to conduct client assessments on a per diem basis . If you have a passion for senior care, a strong clinical background, and the ability to provide personalized care, we’d love to hear from you!

Shifts : Assessments range from 1–3 hours .

Key Responsibilities

As a Registered Nurse with LOES, you will play a vital role in ensuring exceptional care for our clients. Your responsibilities will include :

• Initial Assessments : Completing comprehensive evaluations for new clients to determine care needs.
• Routine Home Visits : Monitoring and updating client care plans during monthly visits.
• PRN (As-Needed) Visits : Responding to client needs with timely assessments.
• Post-Hospital Assessments : Ensuring smooth transitions from hospital to home care.
• Recertification Assessments : Reassessing care needs every six months to ensure optimal support.
• Supervision of Home Health Aides : Overseeing aides during visits to ensure compliance with care plans and quality standards
