We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will be responsible for evaluating, planning, and implementing individualized treatment programs for patients with various physical impairments. This role requires a strong understanding of physical therapy principles, excellent communication skills, and the ability to work collaboratively with other healthcare professionals.
Duties
- Conduct comprehensive assessments of patients to determine their physical therapy needs.
- Develop and implement personalized care plans based on patient evaluations.
- Utilize various therapeutic techniques including trigger point therapy and orthopedic rehabilitation.
- Provide treatment for a wide range of conditions while focusing on geriatrics.
- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
- Collaborate with interdisciplinary teams to coordinate discharge planning and follow-up care.
- Educate patients and their families about treatment options, exercises, and self-management strategies.
- Maintain accurate documentation of patient progress and treatment interventions in compliance with healthcare regulations.
Qualifications
- A degree in Physical Therapy from an accredited institution.
- Valid state licensure or eligibility for licensure as a Physical Therapist.
- Experience in home health, outpatient settings, or acute care preferred but not required.
- Knowledge of systems related to physical therapy practice including care plans and discharge planning.
- Strong interpersonal skills with the ability to build rapport with patients and families.
- Ability to work effectively as part of a multidisciplinary team.
