World Nurses is seeking a travel Physical Therapist for a travel job in San Ramon, California.

Position Overview

We are seeking a skilled and motivated Physical Therapist to join a leading home health team serving Napa and nearby counties. This role focuses on providing personalized in-home physical therapy services to help patients regain mobility, function, and independence in the comfort of their homes.

Key Responsibilities

  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Deliver home-based physical therapy treatments focused on recovery and mobility improvement.
  • Document care plans, progress notes, and updates in accordance with agency standards.
  • Communicate effectively with patients, families, and interdisciplinary healthcare teams.
  • Travel within assigned territories and provide weekend coverage as needed.
  • Maintain compliance with all safety, documentation, and regulatory standards.

    • Qualifications

    Education & Licensure :

  • Graduate of an accredited Physical Therapy program.
  • Active California PT license — Required.
  • BLS certification — Required.
  • Valid Driver’s License , Auto Insurance , and reliable transportation.

    • Experience :

  • Minimum of 1 year of Physical Therapy experience ; home health experience preferred.
  • Must be willing to work with COVID+ or COVID PUI patients if needed.

    • Additional Requirements :

  • Compliance with all health clearances (TB, MMR, Varicella, Hep B, Tdap, Flu, Drug Screen, Physical).
  • Must be flexible with travel and weekend assignments as needed.

    • Workload & Expectations

  • Assigned to multiple counties within designated service areas.
  • Productivity target : 27–30 points per week (each visit type corresponds to defined point values).
  • 36-hour weekly guarantee applies only when productivity and travel requirements are met.
  • Payment structure based on per-visit productivity model ; rate includes drive time, documentation, and communication.

    • Schedule & Assignment Details

  • Contract Length : 13 weeks
  • Shift : Daytime, with potential weekend coverage
  • License : Must be active in California
