Job descriptionAssessing residents upon admission and during ongoing evaluations Communicating significant changes in resident conditions to the interdisciplinary team Documenting assessments accurately and seeking guidance as needed Participating actively in care planning and revising plans as necessary Implementing nursing interventions safely and timely, including medication administration Monitoring resident care and utilizing critical thinking to respond to changes in condition Maintaining professionalism during crisis situations Identifying and addressing problem areas through the QAPI process Focusing on the basic nursing needs of geriatric residents Completing a minimum of twelve hours of nursing-related continuing education annually Graduation from an accredited school of nursing, resulting in LPN licensure Current LPN license in the applicable state CPR and AED certification required, or willingness to obtain within six months IV certification required, or willingness to obtain within six months Preferred experience in long-term care settings Skill in assessing the nursing care needs of elderly residents Strong communication abilities for engaging with residents, visitors, and staff Effective time management and organizational skills Physical ability to stand and walk for extended periods Capacity to lift heavy objects using proper techniques Adept in moving patients and handling equipment safely Manual dexterity and coordination for working with patients and medical equipment Visual acuity for distinguishing color Ability to complete documentation electronically and in writing
