City : Dayton State : OH Start Date : Pending Privileges End Date : TBD Description : Dayton Children's is looking for 24 hour IR coverage starting in September! Open dates : Clinic / Night 29–Oct 3Oct 6–10Oct 27–31Nov 10–14Nov 24–28Dec 15–19Dec 22–26 24-hour weekend 4–5Oct 11–12Nov 1–2Nov 15–16Nov 29–30Dec 20–21Dec 27–28 Schedule : 8-hour day shift Mon-Fri plus weeknight and 24-hour weekend callEMR : EPIC and SentraxAll IR procedures will be turned over to the contract IR physician still to be discussed with DPI but for now assume it will happen except when on vacation. During that time non-angio IR will be provided by DPI.RPI the adult group in town will continue to cover angio for up to 25 of the year which will allow the contract physician time off on weekends when not taking call Must have experience in Angiography amp Embolization 5-6 / day averageMust be IR fellowship trained and able to do all interventional procedures including angiography. Must also have experience and the necessary case logs to cover pediatrics PLEASE NOTE - at time of job acceptance the site requires the following to avoid delays : Confirmation if provider is internally credentialed yes / no DOB SSN Copy of license DEA CSR Life Certifications CAQH Login username password Home Address or address where provider is currently located Cell Agency credentialing contact information must get fingerprinting done immediately after acceptance 1099 Pay Rate : Competitive Market Rates (W-2 Pay Packages available upon request) Certification Requirements : Please confirm credential requirements with VHS upon application.
