POSITION SUMMARY - RN Home Health Case Manager

POSITION DUTIES - Screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the home health patient. Will cover various towns in VT and NH.

Minimum Required Qualifications -

• Compact or VT & NH unencumbered RN license
• Graduate of an accredited school of nursing
• BLS
• 2 years of experience in Home Health

LENGTH OF ASSIGNMENT - 16 weeks

SHIFT / HOURS PER WEEK - Full Time, Days (36hrs / wk guaranteed)

SYSTEMS - Epic

START DATE - 11 / 10 / 2025
