Social Worker- Substance Use Disorder (SUD)

Summary This position may be eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities VA Careers - Licensed Clinical Social Worker : Total Rewards of a Allied Health Professional The Incumbent Is responsible for providing case management and substance use services for veterans In the HUD-VASH program. The incumbent Is responsible for following local policy and Standard Operating Procedures for the HUD-VASH Program. This Includes following policies and documentation requirements as required for Joint Commission accreditation. The Incumbent takes responsibility for Improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other discipline as requested. The incumbent performs accountability functions; 1) document all contacts with patients families ln the medical charts; 2) completes statistical and other reports In accordance with VA Medical Center and respective professional discipline service policies. The Incumbent works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program veterans. The incumbent performs site visits of HUD-VASH apartments to ensure that veterans reside In safe environments In compliance with local housing codes. The Incumbent verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation. The Incumbent ls responsible for ensuring that homeless veterans are considered as primary participants far program acceptance. The Incumbent ls responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures. The Incumbent compiles with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of Interest while serving as the HUD­ VASH Case Manager. The incumbent actively participates in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH.
