Overview :

Sentara Medical Group is seeking an Acute Care Nurse Practitioner to join Sentara's Advanced Heart Failure Clinic in Norfolk, VA . Our teams are dedicated to cultivating an environment where every individual feels recognized, appreciated, and supported, enabling them to perform at their best for themselves and our patients. Our Advanced Practice Providers (APPs) are essential to this foundation, and we seek to continue to grow their impact and scope of practice throughout our hospitals.

Position Highlights :

• Join an experienced team of 7 MDs and 5 APPs
• Primary Location : Sentara Norfolk General Hospital
• Schedule : Mon - Fri, 8 : 00AM - 4 : 30PM
• Inpatient Duties : Rounding on the AHF service line, admissions, discharges, consults, etc.
• Outpatient Duties : Routine AHF follow-ups, lab review, and remote device monitoring (e.g., CardioMEMS, HeartLogic, Cardella, CareSense, etc.)
• Independent panel of up to 14 patients daily
• Call schedule 1 : 6
• 1 year experience required
