LPN Staff - Long Term Care

SUMMARY :

As a Licensed Practical Nurse (LPN), you will play a vital role in providing exceptional care to our long-term and post-acute care residents. Your leadership and organizational skills will guide the work of Certified Nursing Assistants (CNAs) while ensuring that all aspects of patient care are documented accurately and in a timely manner.

RESPONSIBILITIES :

• Administer medications promptly and accurately while documenting all required information for your assigned team.
• Perform medical treatments as prescribed by physicians and follow facility protocols for documentation and monitoring.
• Maintain thorough and precise resident care records using our computerized medical record system to ensure a high standard of care.
• Communicate effectively by giving and receiving written and verbal reports with other staff members as per departmental protocols.
• Monitor residents' conditions closely, reporting any significant changes to the charge nurse or supervisor promptly.
• Conduct pre-reviews of medication administration records in accordance with facility guidelines.
• Complete and timely document Minimum Data Set (MDS) assessments and related observations.
• Support the CNA team by providing assistance with daily activities of living (ADL) for residents when needed.

EDUCATION & LICENSES :

BLS - Basic Life Support - American Heart Association (AHA); LPN - Licensed Practical Nurse - New York State Education Department (NYSED).

PHYSICAL REQUIREMENTS :

Medium Work - Exerting 20 to 50 pounds of force occasionally, or 10 to 25 pounds of force frequently, or greater than negligible up to 10 pounds of force constantly to move objects. Requires frequent walking, standing, or squatting.

PAY RANGE : $28.50 - $34.50

The listed base pay range is a good faith representation of potential base pay for a qualified candidate. It may be modified in the future and is eligible for additional pay components based on experience, qualifications, and internal considerations.
